Who we are
We provide rehabilitation services to individuals who need intensive interdisciplinary rehabilitation services. This is similar to those needing inpatient rehabilitation, but the client can live in the community.
What we do
Our program follows an integrated team approach.
As a patient, you are the centre of this team. You are required to take an active role in your therapy and make decisions about your care. Members of our team help you and your family set goals for improvement in your ability to perform your everyday activities.
Each person’s goals are different. Goals focus on activities or issues that are important to you.
Who we see
We see people aged 18 and older, from across the province, with conditions such as stroke, spinal cord injury, traumatic brain injury, amputations and sports-related injuries.
Our team
Everyone’s needs are different. Your interdisciplinary team may include some or all of the following health-providers:
- Nurses
- Occupational therapist
- Physiotherapist
- Therapeutic recreation specialist
- Psychologist
- Dietitian
- Social worker
- Speech-language pathologist
How to access this service
Patients/clients may be referred to the day rehabilitation hospital by a health-care provider. A completed rehabilitation day hospital referral form (PDF) should be faxed to 709-777-7848. Further questions on rehabilitation day services referrals can be directed to 709-777-6531.
